To expedite the repairs of your yacht, we would appreciate it if you can fill out and sign this form prior to your yacht arriving at our facility.  Please feel free to contact us if you have any questions.

Credit Card Authorization Form (click here)

For your convenience, you can either pay via a check or credit card (MasterCard or Visa only).  We offer a 2% discount if you pay by check.  To expedite the pick up of your yacht, we would appreciate it if you could fill out this form and submit it to us prior to your picking up your yacht.  We do not accept American Express.

If you choose to pay by credit card, please bring your card with you so we can swipe it through our credit card processing machine.

Hurricane Storage Agreement (click here)

We offer dry dock storage for the entire Hurricane Season on a first come first served basis.  Space is limited so you need to make your reservations early.  The attached form is the Hurricane Storage Agreement that you will have to execute.  You will also need to have proof of insurance for your yacht and Apex Marine will have to be listed as additional insured.

Release of Records (click here)

If you are a former customer of Allied Marine, and would like for Apex Marine to have a record of all prior repairs and maintenance work performed on your yacht, Allied requires us to provide the attached written authorization from you.  We will forward the form to Allied Marine.
